SINUS LIFTING KIT:
A special set of bone drills, gum perforators, measuring and auxiliary instruments for lifting the Schneiderian membrane in the maxillary sinus area and filling the maxillary sinus floor with osteoplastic materials in case of insufficient alveolar ridge bone for dental implantation.
The sinus lift kit includes:
1). 30 diamond drills with integrated stoppers for ridge access:
-10 diamond drills with a diameter of 3.65 mm with integrated stoppers from 1 mm to 10 mm;
-10 diamond drills with a diameter of 4.2 mm with integrated stoppers from 1 mm to 10 mm;
-10 diamond drills with a diameter of 5.3 mm with integrated stoppers from 1 mm to 10 mm.
2). 2 round spherical diamond burs for lateral access to the maxillary sinus with a diameter of 4 mm and 5 mm.
3). 9 steel drills with integrated stoppers from 2 mm to 10 mm for from 1 mm to 10 mm - for insertion and compaction of the bone from the ridge side.
4). 3 drills for perforating the attached gingiva of the alveolar ridge with a diameter of 3.65 mm, 4.2 mm, 5.3 mm.
5). Drill extension.
6). Double-sided drilling depth gauge: one side with a diameter of 3.5 mm, the other 5 mm.
7). Straight and angled instrument for working with bone insertion and compaction.
8). Metal syringe for bone insertion.
9). Metal tray for working with bone.
10). Metal kit box - for sterilization in an autoclave.
Advantages:
1- Diamond drills with integrated stoppers of different diameters and heights from 1 mm to 10 mm allow to perform crestal sinus lifting in various clinical cases without violating the integrity of the maxillary sinus membrane.
2- Machine introduction and compaction of bone with drills with integrated stoppers of different heights from 2 mm to 10 mm allow to evenly distribute bone in the maxillary sinus, smoothly lift the membrane, which is impossible to do with osteotomes.
3- Drills have a shortened size for convenient work in the posterior sections of the upper jaw. Also, the shortened size gives less fluctuation of the drill and the hole is more accurate in diameter. If a longer drill is needed, the set includes a drill extension that fits all drills.
4- If at the stage of measuring the drilling depth and checking the integrity of the maxillary sinus membrane, damage to the membrane is found, then the kit includes round diamond drills for opening the lateral window and performing an open sinus lift.

SINUS LIFTING SET:

Indications:
The sinus lift set of instruments is used when the alveolar ridge bone height in the lateral areas of the upper jaw is insufficient for bone preparation, lifting the Schneiderian membrane in the maxillary sinus area and filling the sinus floor with osteoplastic materials to prevent dental implants from penetrating into the maxillary sinus area.
Contraindications:
The operation of lifting the Schneiderian membrane in the maxillary sinus area is widely used for installing dental implants in the maxillary sinus area. Contraindications are all contraindications for dental implantation (see contraindications for dental implantation) and also special contraindications for sinus lifting.
Absolute contraindications for sinus lifting :
① Neoplasms of the paranasal sinuses
② Chronic polypous sinusitis
③ Mucocele
④ Severe allergic rhinitis
⑤ Fungal infection of the paranasal sinuses
Relative contraindications for sinus lifting:
① Chronic sinusitis receiving otolaryngological treatment
② Very small size of the maxillary sinus
③ Mucous retention cyst
④ History of maxillary sinus surgery: high failure and complication rates because the sinus is usually filled with poorly vascularized scar tissue. Sinus lift surgery has contraindications that are classified as relative and absolute, but the decision on surgical technique mainly depends on the surgeon's preference and experience.

Before surgical treatment of patients, it is necessary to carry out general and local preoperative preparation. The goals of preoperative preparation are maximum reduction of inflammatory phenomena in periodontal tissues, soft and hard tissues of the oral cavity, prevention of complications and creation of optimal conditions for restoration of damaged periodontal tissues during and after surgical intervention. Diamond drills lose their cutting ability with repeated use. The loss of cutting ability depends on the density and height of the bone, the technique of use, etc. It is recommended to replace the drills with new ones depending on how they cut the bone.
The set is intended for multiple use. The medical device in the package is not sterile. Before use, sterilization in an autoclave at a temperature of 135 ° C / 274 ° F is required.

OPERATION Crestal sinus lift:
The operation Crestal sinus lift can be divided into several stages:
- checking the necessary instruments and equipment;
- sterilization of the necessary instruments, equipment and premises and preparation of the patient for implantation;
- checking the necessary bone materials, implants and other materials for implantation;
- rinsing the mouth with 0.2% chlorhexidine solution for 2 minutes;
- anesthesia;
- the incision is made only along the alveolar ridge of the posterior part of the upper jaw, without an additional vertical incision. We perform minimal detachment of the alveolar ridge of the bone of the posterior part of the upper jaw. If the height of the alveolar ridge is more than 5 mm, it is possible not to cut, but to perforate the mucous membrane of the alveolar ridge of the posterior part of the upper jaw;
- alternately, with an increase in the height of the working part of the drill, preparation of bone tissue using diamond drills with integrated stoppers until the bottom of the maxillary sinus is opened at a speed of 1500-2000 rpm with mandatory water cooling;
- measuring the drilling depth and checking the integrity of the maxillary sinus membrane. If the integrity of the maxillary sinus membrane is broken, it is necessary to switch to "open sinus lifting". We make an additional vertical incision, lift the flap, expose the lateral bone wall of the maxillary sinus and begin to open the lateral bone window using a round diamond bone drill, which is in the kit and then work as with an open sinus lift (see "open sinus lifting").
- If the integrity of the maxillary sinus membrane is not broken, we collect the bone in a metal syringe and insert it into the prepared bone hole or with an instrument for inserting and compacting the bone;
- using drills for introducing and compacting the bone with integrated stoppers (the height of the working part should correspond to the lip of the bone hole). Counterclockwise movement of the drill (reverse) at a speed of 50-70 rpm without water supply. We introduce from 1cc to 3cc into one bone hole, depending on bone resorption, implant installation, etc.;
- implant placement: depends on the height and width of the alveolar ridge - if the height is above 5 mm, it is always possible, if the height is less than 5 mm, it is necessary to obtain primary fixation and the choice is up to the doctor. The set has 3 diameters of diamond drills - after a diameter of 3.65 mm, we install an implant with a diameter of 4.2 mm, - after a diameter of 4.2 mm, we install an implant with a diameter of 5.0 mm, - after a diameter of 5.3 mm, we install an implant with a diameter of 6.0 mm;
- closing the implant with a screw-cap using a two-stage technique or closing with a gingival format or a one-stage technique;
- suturing the gingival mucosa;
- X-ray after implantation (panoramic or dental);
- observation after implantation surgery;
- opening the implant using a two-stage implantation method.
